M4A LIABILITY WAIVER AND RELEASE FORM

This Liability Waiver and Release Agreement is entered into by and between Mentors 4 Athletes (M4A), a division of the Old Town Athletic Campus (OTAC), and the undersigned participant (the "Participant").

  1. Acknowledgement of Risk
    The Participant understands and acknowledges that participation in M4A may involve physical, emotional, and other risks, including, but not limited to, personal injury, accidents, and emotional distress. By signing this waiver, the Participant voluntarily accepts these risks.

  2. Release of Liability
    The Participant hereby releases, discharges, and holds harmless M4A, its employees, coaches, directors, and volunteers from any and all claims, demands, actions, damages, or causes of action, whether known or unknown, that may arise from the Participant's involvement in M4A, including any injury or harm that may occur.

  3. Indemnification
    The Participant agrees to indemnify, defend, and hold harmless M4A, its employees, coaches, directors, and volunteers from and against any and all claims, damages, or liability arising from M4A’s actions, including but not limited to claims related to personal injury, property damage, or legal actions that may arise due to the Participant’s participation.

  4. Medical Treatment
    The Participant grants permission for M4A to seek emergency medical treatment in the event of an injury or emergency situation and agrees to cover any medical expenses incurred.

  5. Confidentiality
    The Participant agrees to maintain confidentiality regarding any sensitive information shared during M4A and each meeting with a mentee. The Participant will not disclose personal or professional information shared by others in M4A without consent.

  6. Photography and Media
    The Participant consents to the use of their image, likeness, and/or voice in any photographs, videos, or other media captured during M4A for promotional or educational purposes, without compensation or further consent.

  7. Voluntary Participation
    The Participant acknowledges that participation in M4A is voluntary, and they are free to withdraw from it at any time. Withdrawal must be done by notifying M4A Administrative Staff.

  8. Acknowledgement of Understanding
    The Participant acknowledges that they have read and understand this Agreement, and they are signing it voluntarily and without any inducement. The Participant agrees to comply with all rules and policies of M4A.
